Project Objectives / Summary:

The project objective was the development and trial of patient held medical record summaries (PHMRS) as part of the Client Information Assessment and Referral Record (CIARR) being trialled among patients receiving HACC and ACAT services in NSW.  It was submitted that development of such proformas would improve communications between sections of the health and community care sector, which would lead to improved patient outcomes.

The project was to meet the following outcomes:

  • Development of a patient held medical record summary
  • Provide continuity of patient's medication regimes across the primary/secondary interface
  • Improve partnerships between GPs, State and Commonwealth funded health and community service agencies
  • Increase GPs' awareness of home and community care services
  • Develop an enhanced model of care for an aged and/or chronically ill patient population
  • Randomised controlled trial of the use of PHMRS as a part of CIARR
  • Stakeholder assessment of the usefulness of the PHMRS and of the increased liaison between GPs and community health providers

Lessons:

This project was terminated due to a range of circumstances.  There were major organisational changes in the Division resulting in a new incorporated body being established.  Support for the continuation of the project was present initially but this diminished and the project terminated.  There were no indications at the time of assessment of the proposal that there might be such underlying difficulties.
