| Project Objectives / Summary:
This project follows on from an Innovations Funding Pool Round One Project "GP care planning for chronically ill people with a functional disability".
The project aim was to identify and resolve the barriers and limitations of the general practice environment that impedes the effective implementation of the EPC MBS Items.
The primary objective for the project was to develop a self assessment tool that could provide a realistic appraisal of GP and practice readiness to undertake multi-disciplinary care planning. The project also aimed to support EPC staff in Divisions by providing a tool that could identify specific areas where GPs could benefit from further support and guidance regarding multi-disciplinary care planning.
Project Collaboration:
This was a collaborative project between the Central Bayside and Dandenong Divisions of General Practice.
Project Outcomes:
The project had 3 phases:
- The conceptualisation stage
- Piloting a draft "Readiness Index" - 135 GPs responded
- Refinement and finalisation of the Index
The Index has been marketed within the Divisions and to other Divisions to identify the level of readiness for multi-disciplinary care planning with individual GPs and to identify action to increase the uptake of EPC Items. It can be used in a number of ways:
- As a self assessment tool
- As a targeted training needs analysis tool
- As an evaluation tool.
Disseminating Project Information:
"Readiness Index" included on both Divisions' websites.
Lessons:
The project met its stated aims and objectives. While it has obvious usefulness in the earlier stages of uptake of the EPC Items, reapplication of the Readiness Index at a later stage may help to identify levels of change in GP participation in multi-disciplinary care planning. |
